The Cleveland Clinic Community Health Strategy (CHS) is an enterprise-wide initiative to consolidate all community-facing efforts and align resources to provide the highest level of partner-driven, organized, efficient, evidence-based opportunities. These efforts will engage our communities in learning about the impact of the social determinants of health and health inequity on their everyday life.

With the ever-changing landscape in health care and services, the dynamic is changing from “sick” care to “health” care. As such, the health of a community is not determined solely on physical well-being, but on all the factors that go into the experience of living, working and thriving in Northeast Ohio. Cleveland Clinic is committed to working with individuals and organizations that understand this new reality, and these social and environmental determinants will be considered within the framework of any request for support.

The Literacy Cooperative is an umbrella organization that brings foundations, organizations and people together to find practical solutions to improve literacy.  They are mobilizing Northeast Ohio resources to increase reading, math and digital literacy, beginning at birth, so our kids are performing at grade level throughout their educational journey.

Zelie’s Home is a caring community that empowers pregnant and parenting women to take positive and effective action on behalf of themselves and their children by providing physical, emotional, spiritual, and professional support.

The Greater Cleveland Food Bank seeks to bridge the meal gap, connecting individuals with the nutritious meals they need to succeed.  The generosity of our community makes all the difference for those helped by the Greater Cleveland Food Bank’s progra s makes all the difference between having nutritious meals and going hungry.

Womankind provides their free services to women from all over Northeast Ohio who have felt overwhelmed, unsupported of unprepared for their pregnancies.  They provide hope, help and possibility to any pregnant women by offering comprehensive integrative care in a loving, nonjudgemental environment.

We are guided by a common mission, namely seeing the Christ Child in every child, but we respond to the needs of each community in unique ways.  We remain eternally resourceful abiding by one of our guiding principles; “Find a need and fill it.”

Catholic Charities continues the mission of Jesus by responding to those in need through an integrated system of quality services designed to respect the dignity of every person and build a just and compassionate society.  At the heart and soul of the Cosgrove Center is its mission to feed people.

Our Community Outreach Programs are effective vehicles for bringing health education, screening and other health care services directly to those in need.  Our efforts focus on increasing health literacy, providing better access to quality health care facilities and providers, and supporting communities in need.
